Citizen's consent to the processing of personal data received by the Joint Stock Company "Smartavia Airlines"
In accordance with Articles 9, 11 of the Federal Law of July 27, 2006 No. 152-FZ “On Personal Data”, acting freely, of one’s own will and in one’s own interest, confirming one’s legal capacity I give my consent to the joint stock company "Smartavia Airlines", abbreviated corporate name JSC "AK Smartavia" (location: 163053, Arkhangelsk region, Arkhangelsk city, Talazhsky aviation town, Airport street Arkhangelsk, OGRN 1042900018908, INN 2901126323), hereinafter referred to as the Operator, on processing of your personal and biometric data under the following conditions:

2. Consent is given to the processing of personal data without the use of funds automation, and with their use, including collection, recording, systematization, structuring, accumulation, storage (in electronic form and on paper, including number of copies of documents), clarification (updating, changing), use, extraction, depersonalization, blocking, destruction, and any other actions provided for by the current legislation of the Russian Federation, with use of automation tools, including in information and telecommunications networks, or without the use of such means, if the processing of personal data without the use of such funds corresponds to the nature of the actions (operations) performed with personal data using automation tools, that is, it allows search for personal data in accordance with a given algorithm, recorded on a tangible medium and contained in file cabinets or other systematic collections of personal data, and/or access to such personal data data.
3. Consent is given to the processing of the following personal data:
— last name, first name, patronymic (including previous last names, first names and (or) patronymics, if they have changed);
— gender, age;
— date and place of birth;
— citizenship;
— address of actual place of residence and registration at the place of residence and (or) place of stay;
— contact details, telephone numbers, postal and email addresses;
— information about education, profession, specialty and qualifications, including additional education (related to issued certificates; document data about education, qualifications, professional training, information about promotion qualifications);
— information about attitude to military service;
— photographs (appearance without biometric facial recognition);
— marital status, family composition and information about close relatives (including former ones);
— information giving the right to additional guarantees, benefits and compensation;
— information about work experience, including places of previous work;
— information on knowledge of foreign languages, degree of proficiency;
— information about the presence or absence of a criminal record;
— information about the presence or absence of punishment for the use of narcotic drugs or psychotropic substances without a doctor’s prescription, or new potentially dangerous psychoactive substances without a doctor’s prescription;
This consent is valid from the day it is signed until the day the consent is withdrawn in writing.
